Complete and submit the Information Form - the CPRM/CPPM Agreements will follow. On the first page of the License Agreement, please leave the space for the date blank. It will be filled in when the Agreement is countersigned. When completing the License Agreement "Exhibit D" form, please use the space between "a" and ("Adopter") which appears at the top of the page, to indicate the type of business. Examples would be, but not limited to the following: "a Delaware LLC", "a Japanese Corporation", or "a California Corporation". Read and complete the agreement, sign two (2) original copies, and return to the 4C Entity by post. Return signed copies to:4C Entity, LLC To order CPRM or CPPM production keys / MKB's, complete the appropriate order form and return with the signed agreement. Only one technology per order form is acceptable. 4C Entity will send an invoice for the amount indicated on your order form. After receipt of funds, 4C Entity will process your order. 4C Entity order process requires a Public PGP key from an authorized employee. Instruction will be included with the invoice.
